Learning to control your diabetes with lifestyle changes can be challenging. Don’t go it alone. At CentraState, our diabetes educators can provide you with the diabetes management tools and support you need to keep your blood sugar level, feel good, and reduce future complications.
At your initial assessment, we’ll review your medical history and perform a foot exam to assess sensitivity. We will cover proper blood glucose monitoring and provide preliminary meal planning guidance. Your diabetes coach will then create a customized diabetes management program that meets your unique needs, including weight loss, meal plans, and essential resources.
Whether you have just been diagnosed with diabetes or have been living with it for years, you can take control with lasting, healthy lifestyle changes for a sustainable, healthier future.

Prediabetes Treatment
If you’re carrying extra pounds, have high blood pressure, or a cholesterol-heavy diet, you could be at risk for diabetes or have a condition known as prediabetes, where blood sugar levels are higher than normal but not high enough to be diagnosed as type 2 diabetes. Research has found that more than one in three adults has prediabetes—and about 90% don’t even know it. The good news is you can reverse its course with prediabetes treatment.
CentraState’s Prediabetes Treatment Program, recognized by the Centers for Disease Control and Prevention, gives you the tools you need to create lasting, healthy lifestyle changes to prevent diabetes. Our diabetes experts will help you:
- Develop a sustainable, healthier approach to eating
- Increase physical activity by participating in activities you enjoy
- Reach a healthier weight and learn strategies for weight maintenance.

Gestational Diabetes
Our goal is to keep you and your baby safe and healthy. If you are an expectant mother with gestational diabetes, our diabetes educators will provide you with an individualized meal plan and teach you how and when to test your blood sugar. A physician referral or prescription is required to schedule this two-hour appointment.
It is recommended that you also schedule a follow-up appointment after delivery to help reduce your risk of developing Type 2 diabetes.
