CentraState’s MyChart patient portal allows you to take an active role in managing your health. MyChart gives you convenient, secure, 24-hour access to portions of your medical record to help you manage and receive important health information. It can be accessed from any computer, tablet or smartphone that has an internet connection and an up-to-date web browser.

With MyChart, you can:

  • Access test results
  • Request prescription refills
  • View or pay bills
  • Manage appointments
  • Download medical records
  • Communicate with your healthcare providers

CentraState patients can no longer access the previous patient portal. For access to medical records prior to March 13, 2022, you can fill out a request form online. You can request medical records for yourself or someone else. Alternatively, you can print and complete the form in English or Spanish.

How to Create a MyChart Account

If you were treated at CentraState Medical Center for an inpatient stay, visit to our Emergency Department, or for any outpatient service, including lab work, radiology and physical therapy, you have access to your records from these visits through MyChart. There are a few ways to set up your account.

At your next visit or stay, the staff may ask if you are interested in signing up and can send you a personalized link via email to help you do so.

Before you leave the hospital or emergency department, your after-visit/discharge paperwork may also include an activation code that you can use.

If you don’t have an activation code, visit mychart.atlantichealth.org and click on “I do not have an activation code.” After answering questions, you’ll be able to create a username and password and sign in to activate your MyChart account.

Proxy Access allows parents, legal guardians, and caregivers to log into their personal MyChart account, and then connect to medical information for the patient they represent. Available access is based on the age of the patient and proper authorization.
