Every three years CentraState completes a Community Health Needs Assessment (CHNA) to identify the health care needs of the community, in accordance with IRS 501 (r)(3)A). This assessment is based on quantitative and qualitative data gleaned from more than 450 community members through robust surveys and interviews, covering a wide range of topics related to healthcare needs, access and socioeconomic issues. It is supported by secondary data from a number of federal, state and local sources.
The resulting report for 2023-2025 provides the information needed to identify issues of greatest concern so that CentraState can focus resources to those areas and make the greatest possible impact to our community health status, which is outlined in our CHNA Implementation Plan.
